HOSPITALIZATION TERMS
This protection replaces a portion of income if you are hospitalized due to a medical emergency.
The payment is governed by the terms of this membership
Terms with special meanings are defined. See the Definitions section of this Certificate.
Other terms with special meanings are defined where they are used.
1.
Benefit Provisions
1.1 How Payments Start For Hospitalization Protection
To start getting payments from this membership, you must meet all of the conditions listed below, as well as elsewhere in this Certificate:

Your membership is active
-
You are currently hospitalized
-
The first date of hospitalization is after the membership start date

You must provide proof of the hospitalization, as defined in the payout provision section
1.2 When Payments Start For Hospitalization Protection
Your benefits from this Certificate will start on the latest of the dates shown below:

The date your membership starts
-
The date your payout is approved
-
The date you are hospitalized
1.3 When Payments End For Hospitalization Protection
Your benefits from this Certificate will end on the earliest of the dates shown below:

The date your maximum days of payout is met
-
The date your weekly or monthly payment is not made
-
The date you are no longer in hospitalization
1.4 Daily Benefits For Hospitalization Protection
Your daily benefits are 80% of your Daily Income across all the platforms you have on file

Your benefit will be rounded to the nearest dollar.
1.5 You will not receive payment for hospitalization protection if:

You fail to provide any of the information required, as defined in the payout provision section
-
You are not hospitalized
-
You are hospitalized due to a traffic collision
1.6 Case, where the died during the hospitalization, income replacement will only include days up to the day of death if death occurs within the payout window.
2.
Payout Provisions
You must request a payout through the Kover web portal. You cannot request a payout through email or text.
2.1 Proof of qualification
You must link all accounts you work with, such as Uber, Lyft, UberEats, Postmates, Instacart, DoorDash, Wonolo, Amazon Flex, Grubhub, Shipt, Deliv, or Caviar at the time of enrollment on the Kover website.
2.2 Proof for hospitalization protection
You must provide the following information to receive payment:

Length of your hospitalization

Your proof of hospitalization which includes:

Your My Chart Account; or
-
The name of the hospital you are being hospitalized in, the name of your physician, the phone number of the physician and a copy of hospitalization record

A bank account that supports ACH or wire transfer

Any additional information requested by Kover

Other Exclusions
Aside from the exclusions stated in other sections of the certificate, your request will not be paid in any of the following events:

You have a medical condition, injury, or diagnosis that you have received treatment for before your membership start date.

You made a payment request for the same condition related to a new hospitalization.

You are hospitalized due to Labor, Delivery and or complications during labor and delivery

Concealment or Fraud
If you or anyone acting on your behalf has intentionally concealed or misrepresented any information or circumstance that you had a responsibility to disclose, or engaged in any fraudulent conduct, or made any false statements relating to this membership:

Your membership will be voided in the event of any fraud which occurred during the onboarding process

Your membership will immediately be terminated on the date that any fraud occurred during the period of membership
and in either case:

Any fraudulent request or a request which relates to a loss suffered after any fraudulent activity will not be paid

Any money already paid out for requests which are later deemed invalid or fraudulent will be pursued for recovery
-
Law enforcement, legal services, other financial services organizations and anti-fraud databases, as appropriate, will be informed
5.
Right to Termination
Kover reserves the right to terminate any member’s account if the user does not follow user guidelines.
DEFINITIONS
1.
Hospitalization
A member must be an inpatient to be considered hospitalized. Inpatient care is the care of patients whose condition requires admission to a hospital for an overnight stay in the hospital. Note that just going to the Emergency Room does not mean you are being hospitalized.
2.
Membership start date
Membership start date is the date your membership turns active and you receive your activation email.
3.
Daily Income
Daily Income is the average income you earned over the 30-day period that you make the most month in the last 90 days. If you have less than 30 days of data, then the daily income is the average income you earned over the period that you work for the platform.
4.
Maximum Days of Payout
Maximum days of payout is the maximum days you selected during the enrollment process.
5.
Member
Member is defined as one individual named on the membership. The membership cannot be shared with or transferred to another individual.
